Where can I find all current Utah Secretary of State business forms?

All Utah business entity forms are published on the Business Entities portal of the Division of Corporations and Commercial Code. The portal organizes forms into Corporations, Limited Liability Companies, Partnerships (including General Partnerships, Limited Partnerships, Limited Liability Partnerships, and Limited Liability Limited Partnerships), and Miscellaneous categories (Doing Business As, Business Trust, Commercial Registered Agents, Decentralized Autonomous Organizations, Tribal Business Entities, Uniform Limited Cooperative Associations, and Collection Agencies).

Online submissions are processed through Fast Track Filing, which requires a free UtahID account. The Fiscal Year 2026 Fee Schedule, effective July 1, 2025, is the controlling source for every fee cited on the portal.

Is the registered agent consent form required to be filed with the Utah Secretary of State?

Utah does not publish a separate registered-agent consent form. Consent is captured within the formation, foreign registration, or Registration Information Change document itself, where the registered agent’s signature or written acknowledgment appears on the face of the filing or as an attached authorization.

A copy of the agent’s signed consent is retained internally by the represented entity rather than filed as a standalone document with the state. Designating an agent without the agent’s actual consent exposes the filer to risk that the appointment will be rejected by the Division and that service of process may fail.

What is the difference between the entity-filed change form and the agent-filed change form in Utah?

The entity-filed Registration Information Change Form is filed by the business itself, accepts a single designator at a time, and triggers a $17 fee per affected entity; it is the appropriate form when only one represented entity needs an update.

The agent-filed Commercial Registered Agent Change Filing is filed by the Commercial Registered Agent itself, updates the agent’s roster of represented entities in a single filing rather than entity-by-entity, and triggers a flat $52 fee regardless of how many represented entities are on the roster. The Commercial Registered Agent must provide advance written notice to each represented entity before the change takes effect.

Utah does not maintain a non-commercial registered-agent update form; individual (non-commercial) agents must coordinate changes with each represented entity so that the entity files its own Registration Information Change Form.
